Our board certified pediatricians would like to work with you as you raise healthy children and adolescents.  Our physicians, nurses and office staff are committed to providing you with the finest pediatric care available.

In order to save time when you arrive, we ask that you please take the time to fill out our new patient forms before your visit, available when you click on the "Forms & Policies" tab.  Please bring your current insurance card and ID or Drivers License with you.  If you are transferring from another practice, we will need copies of your child's medical records, especially vaccinations.  You will need to sign a release of records for us to obtain these.  We prefer to have your child's medical history prior to scheduling an appointment. 

We accept most insurance companies and will submit your claims if we are a provider.  Please consult your insurance plan to confirm if we are in your network.  Medicaid (Buckeye, Paramount and regular Medicaid) is accepted by our office; please contact our office to check eligibility requirements.

At the end of your child's visit you will be given an encounter form to take to the cashier.  The total charged will be based on services rendered during the appointment.  The insurance companies that we contract with will require us to accept your co-payment which is due at the time of the service, should you have a co-pay at time of service.  If you are self pay, payment is required at the end of the visit.  We do access a $15.00 fee for form completion that includes Sports Exams, Preschool, Day Care and Boy and Girl Scouts and Work Physicals to allow participation.  We will submit this to the insurance company you have provided us with.

We accept Visa, MasterCard, Care Credit and personal checks and expect payment from the parent accompanying the child to the visit.  If an absent parent has financial responsibility, we must ask for payment from you and allow you to make arrangements with the responsible party.
